About proof of vaccination

If you received 1 or more doses of a COVID‑19 vaccine in Ontario, you will be able to download a vaccine certificate with QR code. Your vaccine certificate is an electronic record of your COVID‑19 vaccination, including information about each dose, which can serve as a personal health record and/or proof of vaccination.

Ontario no longer requires businesses and organizations to check for proof of vaccination. However, individual businesses and organizations may choose to require proof of vaccination upon entry, on their own volition, providing the requirements or policies comply with all applicable privacy laws, including theOntario Human Rights Code. Businesses may choose to seek independent legal advice on continuing these measures on a voluntary basis.

Individuals may also be required to show proof of vaccination for some businesses, settings or workplaces or when travelling within and outside of Canada. Ontario’s definition of being up to date with vaccinations may differ from what is required for these settings, workplaces or international travel.

Check applicable requirements at your destination and with your airline, railway company or travel agent. You may also find more information ontravel requirements from the Government of Canada.

How to get proof of vaccination

If you have a green photo (OHIP) health card

You can download your vaccine certificate from Ontario's vaccination portal. To log in securely, you need:

  • a green photo health (OHIP) card (you will need numbers from both the front and back of the card, expired cards will be accepted)
  • your date of birth
  • your postal code (the one associated with your health card)

If you do not have a computer or need help, call the Provincial Vaccine Contact Centre to request a copy of your vaccine certificate by email or mail.

You can also:

  • visit your local library to use a public computer and internet resources
  • ask a trusted friend or organization to help print a copy
  • access a ServiceOntario location

If you have a red and white health card

Call the Provincial Vaccine Contact Centre and the contact centre agent can email you a secure URL to access your vaccine certificate.

If you don't have an Ontario health card

Call the Provincial Vaccine Contact Centre and the contact centre agent can email you a secure URL to access your vaccine certificate.

If your information is not in the provincial database

Contact your local public health unit or the clinic where you received your dose(s).

Get help from the Provincial Vaccine Contact Centre

Agents at the Provincial Vaccine Contact Centre (PVCC) are able to assist you with your vaccine certificate.

Registering out of province vaccinations

If you need to record a COVID-19 vaccine received outside of Ontario or Canada, you should contact your local public health unit.

Recording out-of-province or out-of-country doses is important to ensure future COVID-19 vaccine doses can be given safely and at the appropriate time intervals.

Out-of-province doses should be recorded if:

  • a child is younger than 5years old and has received1 or more doses outside of Ontario, to ensure they have completed the appropriate number of doses for optimal protection
  • you are aged 5years and older and have received 1 or more doses outside of Ontario within the last 6months, to ensure future doses (if needed) can be given at the appropriate interval

Other doses can be recorded at the discretion of the local public health unit.

To do this, you shouldcontact your local public health unit to have your COVID-19 immunization record documented in COVaxON.

Once recorded, you will be able to book additional doses (if needed) and obtain a vaccine certificate through the provincial portal atHow to get your COVID‑19 vaccine certificateor by calling the Provincial Vaccine Contact Centre.

If you have applied to have your out of province vaccination(s) registered with yourpublic health unitand are waiting to have them uploaded, you can call the Provincial Vaccine Contact Centre to book your next vaccine appointment if you fit the following criteria:

  • you have an Ontario health card or COVID ID (a unique number assigned to you by your public health unit in place of an Ontario health card number)
  • your public health unit uses the provincial booking system
  • you are able to bring your out of province vaccination receipts to your vaccination appointment for validation

Medical and COVID-19 vaccine clinical trial exemptions

New and renewed medical and COVID-19 vaccine trial exemptions will no longer be recorded in your vaccine certificate. Existing medical exemptions will appear on your vaccine certificate for 6 months from your last exemption.

If your personal circ*mstances require that you have a medical exemption (such as for work or travel), you should follow up with your health care provider or your employer’s occupational health and safety team to request or validate the necessary documentation.

If you require a COVID-19 vaccine trial exemption for your personal circ*mstances (such as for work or travel), you should follow up with your vaccine trial Principal Investigator or Study Director to obtain the necessary documentation.
